Patient Information |

New Patients |
- Please download and print the "New
Patient Form". This required paperwork needs to
be filled out in its entirety and brought to your initial
physical therapy visit.
|
In Preparation |
- Please dress comfortably for your appointments. You will
be required to complete some
new patient paperwork,
which can be filled out just prior to your initial
appointment. You should arrive approximately 15 minutes
early in order to be ready for your appointment time with
your therapist.

Insurance/Finances |
- On the first visit, you will receive a personalized
insurance consultation session where all of your financial
questions will be answered. By the close of the session, you
should be comfortable with costs and available payment
options offered by Premier Physical Therapy. It is our goal
to meet your financial needs so that money will never
interfere with your recovery.

First Clinical Visit |
- After your insurance consultation, you can also expect a
thorough, interactive evaluation from a Licensed Physical
Therapist in order to establish a customized treatment plan
that will assist you in meeting your functional goals.
During this evaluation you will have the opportunity to
discuss your condition, help set goals for your therapy, and
gain a good understanding of what you may expect from
attending physical therapy visits including prognosis,
treatment interventions and approximately how long it should
take.

Follow Up Clinical Visits |
- On all follow up visits, you simply sign in at the desk
in the lobby and expect to wait no longer than 5 minutes for
a qualified therapist to assist you to the treatment area.
From here you will continue your treatment progression.
Expect approximately 60 minutes per treatment.

Physician Communication |
- Communication with your referring physician is an
integral part of your therapy to ensure consistent quality
care as changes and progression occurs. You should expect a
Physician Progress Note to be sent with you each time you
return to your physician.
